Clinical Lab 2.0:
How the Laboratory Can Support Value Based Care, Optimize Patient Outcomes, and Reduce Total Costs of Care?

Dr. Grenache is the chief scientific officer for TriCore Reference Laboratories in Albuquerque, New Mexico. In that role, he leads the TriCore Research Institute which performs clinical device trials, offers central lab services, and maintains asample biorepository.

He is also the laboratory director of TriCore’s core laboratory, is the medical

director of immunology and point-of-care testing, and a clinical professor of

pathology at the University of New Mexico. He is the immediate past president of the American Association for Clinical Chemistry.

Dr. Grenache earned his PhD in biomedical sciences from WPI in Worcester,

Massachusetts, and completed postdoctoral training in clinical chemistry at

Washington University School of Medicine in St. Louis, Missouri. He is a diplomate of the American Board of Clinical Chemistry and a Fellow in the AACC Academy. His research interests are centered on leveraging longitudinal laboratory data to gain insights into population health and the diagnostic tests used to manage the pregnant patient.

The next IFCC webinars: “Ethical dilemmas in laboratory medicine during pandemic”  will be held on 20 October, 2021 and “Common laboratory measures of dysglycaemia; Their strengths and weaknesses” will be held on 27 October, 2021.

  • 20 October 2021:  “Ethical dilemmas in laboratory medicine during pandemic” With its Task Force on Ethics, the IFCC creates a collective voice for ethical issues in Laboratory Medicine and raises awareness of ethics-related issues. The COVID 19 pandemic tests our professional responsibility and demands that we do not simply perform tests and use technology uncritically. In this webinar we will first talk about the causes of the profoundly reduced life expectancy Australia’s remote-dwelling Aborigines and describe demands upon the medical practitioner that are so fundamental that they are rarely encountered in “urban” medicine. Then we will concentrate on how the Covid-19 pandemic is changing the research and society, focusing on the use of remnant clinical samples for research, often exempted from informed consent and the idea of collaboration with scientific research as a moral duty. Finally, as this pandemic has emphasized many long-standing inequities in communities we will highlight the impact of long-standing medical conditions, challenges of testing and contact tracing, mistrust of medical institutions and vaccines.   • 27 October 2021: “Common laboratory measures of dysglycaemia; Their strengths and weaknesses” Diabetes Mellitus, and its precursor clinical syndromes, continue to grow in prevalence in developed and developing countries across the world. Until we have better strategies for prevention, we rely on laboratory testing to screen, diagnose and manage these conditions. Each of these laboratory tests; glucose, insulin and HbA1c play vital roles in understanding and managing diabetes and over several decades laboratory professionals have improved their individual strengths, but also identified some significant weaknesses. No laboratory test is perfect, but it is vital that laboratory professionals are aware of any weaknesses and try to mitigate that risk so that the guidance that the laboratory provides to the clinician, and ultimately the patient, can be clear and balanced. This seminar will allow the participant to identify the weaknesses with the measurement and reporting of glucose, insulin and HbA1c, but also to recognize the laboratory techniques to minimize the risks associated with these weaknesses.

The next IFCC webinar: “Evidence-based Laboratory Medicine” will be held on 8 Sept, 2021.

It has been stated that the laboratory assists in 60-70% of all clinical decisions, e.g. admission, discharge and drug therapy. The goal of the laboratory is to provide information that is useful in clinical decision making and to prevent cases where incorrect interpretation leads to poor patient outcomes.

Evidence-based medicine (EBM) has been described as the “conscientious, explicit, and judicious use of current best evidence in making decisions about the care of individual patients”. It involves the integration of best research evidence with clinical expertise and patient values. Evidence-based laboratory medicine (EBLM) focuses on the evaluation and use of laboratory tests to improve patient outcomes. It assists clinical management by integrating the best available research evidence for the use of laboratory investigations with the physician’s clinical expertise and the needs, expectations and concerns of patients. This leads to improved care and outcomes of individual patients and the effective use of healthcare resources. Evidence-based guidelines are developed using the best evidence to develop statements that guide decisions and criteria regarding diagnosis, management, and treatment in specific areas of healthcare.

In this webinar, we will cover the basic concepts of EBLM and the “5A’s” involved in obtaining this evidence. Then we will discuss how guidelines are implemented using the best available evidence. And finally, we will look at evidence to see whether point of care testing confers any advantage in clinical decision making in different scenarios.
